Please ensure you download the correct form, and we appreciate you returning it to us either by email, fax or drop off before the day of your assessment or follow up appointment.

Attention Deficit Hyperactivity Disorder (ADHD) is a term used to describe and encompass multiple diagnoses, including inattentive and hyperactive subtypes. Please review the application in detail and determine whether your child meets criteria BEFORE asking your doctor to complete the form. It is extremely helpful for you to provide examples of impairments and report the severity and impact of the illness for the doctor to complete the form.
